• Name of licensee
• License status (current or forfeited and where applicable, active or inactive)
• Date initially licensed
• Business addresses (see Disclaimer on next page for further clarification)
• Employment (for license areas that require reporting of employment or association)
• License class (contractor licenses only)

By law, only public information may be disclosed.

The lists generated will be comma-separated text files (.txt files). These files can be easily loaded into most database formats (e.g. Access, Excel, Oracle)."
